Application forms are available at the NTS website, www.nts.org.pk which must read carefully before applying for any position. Candidates are required to use their prescribed application form; and NTS test fee slip along with CNIC and recent passport size photographs to Health Department Khyber Pakhtunkhwa DHIS (Project) National testing service-Pakistan, Headquaters:1-E, Street #46, Sector 1.812, Islamabad. NTS will charge Rs. 400, per application form the candidates for the whole process.
The form should be carefully filled in by the candidate as the provided information will be considered as final for further processing.
In case of wrong apply or not meeting any of the required criteria candidates will be responsible and can be excluded from the process even atter provisionally allowed in the test in case of wrong apply.
Not fulfilling any of the written statements of the application form or any MIS-InfOMffition Will automatically exclude the candidate from the recruitment process at any stage.
NTS test will serve only for the screening !shortlisting purpose &post test process will be performed by the DHIS itself In accordance with the relevant rules, regulations, and procedures.
